Twins’ Deaths: Lagos Begins Autopsy Probe After Post-Vaccination Tragedy
The Lagos State Government has ordered a post-mortem examination and toxicology tests following the death of nine-month-old twins who reportedly died less than 24 hours after receiving routine immunisation at a government health facility in the Ajangbadi area of Ojo Local Government Area.
The twins were vaccinated on December 24, 2025, at the Ajangbadi Primary Health Centre and were said to have developed complications later that day. They reportedly died on the morning of December 25, sparking public concern and intense debate on vaccine safety and healthcare standards in the state.
Confirming the development, officials of the Lagos State Primary Health Care Board said a comprehensive investigation had commenced, including an autopsy, laboratory analysis and a review of the vaccines administered, storage conditions and procedures followed by health workers at the facility.

According to the children’s father, the twins appeared healthy before the immunisation but became weak and lethargic hours later. He alleged that despite administering recommended medication, their condition worsened, leading to their deaths. His emotional account, shared on social media, has drawn widespread reactions and calls for accountability.
In response, the Lagos State Government said the matter had been reported to relevant authorities, including security agencies, assuring the public that findings from the investigation would be made public once concluded.
Health officials stressed that routine childhood immunisation remains safe and critical in preventing deadly diseases, noting that millions of children receive the same vaccines annually without adverse outcomes. They cautioned against speculation, urging the public to await the outcome of the post-mortem results.
The incident has renewed calls for greater transparency in the health sector, improved communication with parents, and strict adherence to vaccination protocols across public health facilities in Lagos.
